G's Story
G is a 78 years old male. He was admitted to rehab post an Ischaemic Stroke with cognitive communication difficulties. He was admitted for 16 weeks.

'Activity Planning' was used to restore cognitive-linguistic skills to better organise, sequence, and plan tasks, and to improve functional communication and daily living activities. G was particularly keen to plan a long-distance tranin joutney so he can go and visit family.
